what is the long term outlook on someone who has copd and coronary heart disease?

Depends on age and the extent of their condition as well as other general health.COPD is progressive and patients generally deteriorate over time but the natural history of the disease varies in different people.
If you were a smoker, providing you are no longer exposed to cigarette smoke, Medication can slow the rate of the declining lung function.
